Puxico Nursing & Rehab Center is a 60-bed licensed Medicare and Medicaid certified facility that provides 24-hour care to our Residents. We strive to give you and your family members the best care possible 365 days a year.  Our team of professionals provide high quality nursing care in a clean, friendly, and comfortable environment.  Our staff is committed to our philosophy that each and every Resident should reach and maintain the highest level of functioning and be as happy and independent as possible.  We provide skilled services for rehab to home, short stays, or on a long-term basis from our excellent Therapy Department. We are able to provide Physical, Occupational, and Speech therapy. We offer transportation to and from physician visits and also provide a full calendar of activities that are sure to meet the needs of our residents. Our social service department works hand in hand with each Resident and their family to provide a smooth transition to our facility and assist with any financial assistance that may be needed.

Admissions are accepted 24 hours per day, 7 days a week.
